San Diego County Probation operates the Community Transition Center (CTC), established 13 years ago as California's only centralized reentry facility that provides comprehensive support to individuals immediately after their release from prison. The center serves approximately 1,000 people annually on post-release community supervision by offering integrated services including behavioral health assessments, housing assistance, substance abuse treatment, and case management through a multidisciplinary team approach. Since its inception, the CTC has processed over 23,800 individuals, prevented more than 2,400 people from returning to custody, and saved over 17,100 jail days while supporting long-term public safety goals.
